The healthcare provider may grant a discount, if required.
NEUROLOGICAL NON-PUBLIC HEALTH CARE CENTER shall not levy any charge for providing medical records to patients.
The postage fee for sending medical documentation (registered letter with acknowledgment of receipt) is paid by the patient, according to the price list of Poczta Polska (Polish Post).
